How is HCl secretion promoted?

1. Cephalic phase: it involves the anticipation of food (CNS) and release of ACh from the vagus nerve (PSNS). The stimulation of muscarinic receptors on parietal cells and G cells lead to secretion of HCl into lumen and promotion of HCl production

2. Gastric phase: it involves the chemoreceptors and stomach distension triggering the ENS and release of ACh. The stimulation of parietal and G cells and increased pH via food bolus leads to more gastrin and HCl secretion

ACh and gastrin also stimulate mast cells and enteroendocrine cells: releases histamine which activates H2 receptors on parietal cells.

How is HCl secretion inhibited?

1. PH falls to 1-3:
  • G cells are inhibited, thus less gastrin, thus less HCl secretion by parietal cells
2. Presence of chyme in duodenal lumen
  • Somatostatin secretion, thus less HCl secretion by parietal cells
  • Acidification of duodenum leads to secretion of secretin, leading to exocrine pancreas and gall bladder to secrete bicarbonate into small intestine & further somatostatin and PGE2 release
  • CCK secretion into bloodstream leads to release of pancreatic digestive enzymes into duodenum
